Silent Hill 2 Remake randomises several of its puzzle solutions and changes others entirely between difficulty settings. So a walkthrough that gives you a code is often giving you somebody else's code, and the useful thing is the method rather than the answer.
Every solution below says which difficulty it was recorded on. Check that first. If it says Standard and you are on Hard, expect the shape of the puzzle to hold and the specific answer to differ.

The two that stop people for hours
The Moth puzzle in Blue Creek. It has a different presentation on Easy than on Standard, which is why two people describing it to each other end up arguing. Confirm your difficulty before you confirm your answer.
The Hangman poem in the Labyrinth. It is a reading puzzle rather than a searching one, and the poem means what it says. People fail it by looking for a trick that is not there.
The collectible areas, all fourteen
Memos, photos and glimpses are tracked per area, and the areas are numbered by the game.
| # | Area |
|---|---|
| 1 | South Vale, East |
| 2 | Blue Creek Apartments |
| 4 | South Vale, West |
| 5 | Brookhaven Hospital |
| 6 | Brookhaven Hospital, Otherworld |
| 7 | South Vale, Otherworld East |
| 8 | South Vale, West Revisited |
| 9 | Silent Hill Historical Society |
| 10 | Toluca Prison |
| 11 | Labyrinth |
| 12 | Toluca Lake |
| 13 | Lakeview Hotel |
| Final | Lakeview Hotel, Otherworld |
South Vale appears four times as four separate areas: East, West, Otherworld East and West Revisited. They have different contents, and treating them as one place is the most common reason a collection run comes up short.
The 26 Strange Photos
Strange Photos drive the Pieces Unarranged trophy, and there are 26 of them. Chapter 4 has its own route because that chapter holds several and is where people first notice they are behind.
The photos are a key item rather than a memo, so they do not appear in the same list as the rest of your collection. Check both.
The trophies, and which need planning
| Trophy | Needs planning? |
|---|---|
| Pieces Unarranged, all 26 Strange Photos | Yes |
| Archivist | Yes, it is the full collection |
| Echoes | Yes |
| Radio Silence | Yes, turn the radio off deliberately |
| Scourge of Toluca Lake | Yes |
| James of All Trades | Yes |
| All Seems in Order | No |
| Nice and Cozy | No |
| No Turning Back Now | No |
| Enjoy Your Stay | No |
| You Never Know | No |
| Let's NOT Party | No |
| Leftovers | No |
| It's Bread | No |
Radio Silence is the one people trip over, because turning the radio off is something the game actively discourages you from doing. It has its own guide for that reason.
The way to play it
- Set your difficulty and remember it. Puzzle difficulty is separate from combat difficulty in this game, and the puzzle setting is the one that decides whether any solution you read applies to you.
- Sweep each area before you leave it. Silent Hill 2 lets you back into very little.
- Treat the four South Vale visits as four areas.
- Track Strange Photos separately from memos and photos, because the game does.
- Turn the radio off at some point, deliberately, for Radio Silence.
Every puzzle above has a video showing the solved state rather than describing it, which matters here more than in most games: several of these are visual arrangements, and a paragraph describing where a radiograph goes is worse than one second of footage.
